Before Your First Visit

To save time at your first appointment, please download, print, and complete your registration form in advance. Return the signed form to us before your visit using any of the options below.

- Photo ID (driver's license or state ID)
- Insurance card(s)
- Any prior dermatology records or pathology reports if available
Insurance & Payment
Peterson Dermatology accepts most major insurance plans including Medicare and Medicaid. Please call our office at 912-538-9080 to verify that we accept your specific plan before your appointment. We also offer self-pay options — please ask our staff for details.
Office Policies
Appointments
All new appointments require the new patient paperwork be completely filled out and returned to us before an appointment can be made. If you need to cancel or reschedule, please call us at least 24 hours in advance.
After-Hours Emergencies
Please go to the nearest Emergency Room for after-hours emergencies.
Prescription Refills
Prescription refill requests require an office visit within the past 12 months. Reach out to your pharmacy to send the refill request, then allow 48–72 hours for refill processing.
Referrals
If your insurance requires a referral, please obtain one from your primary care physician prior to your appointment.
